Mechanism of Action
Ertapenem is a broad-spectrum carbapenem antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins. This action disrupts cell wall integrity, leading to bacterial cell death. It exhibits activity against a wide range of gram-positive, gram-negative, and anaerobic bacteria.

Indications
Ertapenem is used to treat various infections, including:
Complicated intra-abdominal infections
Complicated skin and skin structure infections, including diabetic foot infections
Community-acquired pneumonia
Complicated urinary tract infections, including pyelonephritis
Acute pelvic infections, such as postpartum endomyometritis and septic abortion
Prophylaxis of surgical-site infections following elective colorectal surgery
Diabetic foot infections

Dosage and Administration
Adults and Adolescents (≥13 years):
1 gram IV or IM once daily
IV infusion should be administered over 30 minutes
IM injection is typically given in the gluteal muscle
Pediatric Patients (3 months to 12 years):
15 mg/kg IV or IM every 12 hours, not to exceed 1 gram per day
Renal Impairment:
For creatinine clearance (CrCl) <30 mL/min or end-stage renal disease, the recommended dosage is 500 mg IV once daily.
If ertapenem is administered ≤6 hours before dialysis, a supplemental dose of 150 mg should be given after dialysis. ([Medical Dialogues][6])

Changes in some laboratory blood and urine tests

Decrease in white blood cells or platelets
Low blood sugar
Agitation, anxiety, or depression
Irregular heart rate or increased blood pressure
Bleeding or fast heart rate
Nasal congestion, cough, or pneumonia
Inflammation of the gall bladder or liver disorder
Skin peeling or infection of the wound after surgery
Muscle cramp or shoulder pain
Urinary tract infection or kidney impairment
Miscarriage or genital bleeding
Allergy or feeling unwell
Pelvic peritonitis or fainting
Hardening of the skin at the injection site

Contraindications
Ertapenem is contraindicated in individuals with known hypersensitivity to ertapenem, other carbapenem antibiotics, or any component of the formulation. Cross-sensitivity with penicillins and cephalosporins may occur. Use with caution in patients with a history of severe allergic reactions to beta-lactam antibiotics.
